If you are reselling Conklin products, you must fill out the appropriate Sales/Use Tax Exemption Certificate and send them to our Finance Department.
Many of the forms are available as fill-in PDFs, which allows you to type in your information. You then may print, email, fax, or mail the document. You may also print a blank document and handwrite your information.
Remember to add your Conklin Account number to the top right corner of your form before submitting.

Uniform Sales and Use Tax Certificate has been developed that 38 States (Including the District of Columbia) have indicated is acceptable for use as a blanket exception certificate. The certificate itself contains instructions on its use, lists the states that have indicated that a properly filled out form satisfies their requirements for a valid exception certificate, and sets forth specific limitations on it use. Please use this form to file your tax exemption certificate with Conklin Company, Inc.
